What is azithromycin?
Azithromycin is a common macrolide antibiotic used to treat some bacterial infections including bronchitis, pneumonia, STDs, and ear, lung, sinus and throat infections. It is often used to treat infants and those with weaker immune systems. Azithromycin prevents bacteria from growing in the body by interfering with its protein synthesis to inhibit growth.
How do I take azithromycin?
Azithromycin usually comes as a tablet but can also be prescribed as a liquid. The tablets and the liquid are to be taken orally, usually without food, once a day for 1 to 5 days. The body is better able to absorb the antibiotic on an empty stomach. To help you remember to take azithromycin, take it around the same time every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take azithromycin exactly as directed.
To take the liquid azithromycin, shake it well before each use to ensure that it is mixed evenly. Use a dosing spoon, oral syringe, or measuring cup to measure the correct amount of medication. Rinse the measuring device with water after taking the full dose of medication. Use the extended-release liquid within 12 hours of receiving it from the pharmacy.
The extended-release liquid is usually taken on an empty stomach (at least 1 hour before or 2 hours after a meal) as a one-time dose. If you receive the extended-release azithromycin as a dry powder, water must first be added to the bottle before taking the medication. Measure 1/4 cup (60 mL) of water and add it to the bottle. Close the bottle tightly and shake well to mix. Use the extended-release liquid within 12 hours of adding water to the powder or throw it out.

When will I feel better?
You should begin to feel better during the first few days of treatment with azithromycin. If your symptoms do not improve, or get worse, call your doctor. Take azithromycin until you finish the prescription, even if you feel better. If you stop taking azithromycin too soon or skip doses, the bacteria may not be completely eliminated, and the infection may return even stronger with a resistance to the antibiotic treatment.
How do I store azithromycin?
It is best to keep this medication in the container it came in. Keep the container tightly closed and out of reach of children. Store azithromycin tablets and liquid at room temperature, away from excess heat and moisture. Do not store in the bathroom as the medication may be exposed to excessive moisture. Do not refrigerate or freeze the suspension. Throw away any unused azithromycin liquid after dosing is complete. Talk to your pharmacist about the proper disposal of your medication.
Before taking azithromycin, tell your doctor if:
- You are allergic to azithromycin, clarithromycin (Biaxin, in Prevpac), dirithromycin, erythromycin (E.E.S., E-Mycin, Erythrocin), telithromycin (Ketek), or any other medications.
- You are taking anticoagulants (blood thinners) such as warfarin (Coumadin, Jantoven), cyclosporine (Neoral, Sandimmune), digoxin (Lanoxin), dihydroergotamine (D.H.E. 45, Migranal), or ergotamine (Ergomar).
- You are taking medications for irregular heartbeat such as amiodarone (Cordarone, Pacerone), dofetilide (Tikosyn), procainamide (Procanbid), quinidine, and sotalol (Betapace, Sorine), nelfinavir (Viracept), phenytoin (Dilantin), and terfenadine.
- You have ever had jaundice or other liver problems.
- You have low levels of magnesium or potassium in your blood or if you have a blood infection.
- You have or have ever had a fast, slow, or irregular heartbeat.
- You have or have ever had cystic fibrosis, AIDs or HIV, myasthenia gravis, or kidney or liver disease.
- You are pregnant, plan to become pregnant, or are breastfeeding.
If you take antacids (Mylanta, Maalox), take them 2 hours before or 4 hours after azithromycin tablets or liquid. The extended-release liquid may be taken at any time with antacids.
Azithromycin may be particularly risky for those with heart problems, as it can lead to potentially fatal irregular heart rhythm. Talk to your doctor about amoxicillin or another alternative antibiotic.
It is important for you to keep a written list of all of the prescription and nonprescription medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. Bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.
What if I forget to take a dose of azithromycin?
Take the missed dose of azithromycin as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue on your regular schedule. Do not take a double dose of azithromycin to make up for a missed one. Be sure to finish all the azithromycin that is prescribed by your doctor (in evenly spaced doses) as bacterial infections can return or develop a resistance to azithromycin.
What are the possible side effects from taking azithromycin?
Azithromycin may cause the following side effects which are mostly gastrointestinal. Tell your doctor if any of these symptoms are severe or do not go away:
- nausea
- diarrhea
- vomiting
- stomach pain
- heartburn
- headache
Azithromycin may cause more severe side effects on rare occasions such as wheezing and anaphylaxis as well as other allergic reactions such as swelling, hives, rash or itching. Other severe side effects include fast or irregular heartbeat, dizziness, fainting, severe diarrhea, unusual bleeding or bruising, dark-colored urine, flu-like symptoms, weakness or tiredness and hoarseness.
Azithromycin may cause other side effects. If you experience a serious side effect, you or your doctor may send a report to the Food and Drug Administration's (FDA) MedWatch Adverse Event Reporting program online [at http://www.fda.gov/Safety/MedWatch] or by phone [1-800-332-1088].
What happens if I overdose on azithromycin?
In case of an overdose on azithromycin, call your local Poison Control Center at 1-800-222-1222. If the victim has collapsed or is not breathing, call local emergency services at 911.
